DSP OF THE MONTH AWARD
 NOMINATIONS

Each month, employees are provided the opportunity to nominate their teammates to be recognized for their outstanding contribution to Community Living of Rhode Island. The DSP of the Month is someone who exemplifies excellence in their work performance, while providing exceptional support to the individuals we support.

How to Nominate a DSP

The nomination process is completely online and requires that you complete the form below. All questions must be answered for your vote to count!

Who is Eligible to be nominated? 

All full-time and part-time regular employees are eligible to participate in the Employee-of-the-Month Program. Per Diem & New employees are ineligible to participate in the employee of the month program. For new employees, you will have to maintain 6 months of continuous employment with CLRI to be selected as employee of the month or to nominate a co-worker. An employee may be designated as Employee-of-the-Month only once during a calendar year.

NOMINATION

FORM

The last day to submit your nomination is the 20th of each month.

Comes to work at a minimum of 5 minutes before his/her resumption time to conduct a proper handover and wellness check on all participants in the program.
This employee read and initial every single communication log entry, including completing behavior tracking forms, incident reports form, ISP tracking forms, daily status notes, and all other program supporting documents.
This employee cleans the program thoroughly, cleans the van after each use, assists the participants in cleaning their rooms, laundry, and other activities of daily living.
This employee maintains a positive rapport with the program participant. Maintain the safety of participants at all times, respect participants' rights & dignity, apply sound judgement at all times, creative with recreational activities, and conduct a wellness check on the participants every 30 mins.
Apply sound judgement in effectively de-escalating challenging behaviors, showing up on time for all meetings, including mandatory training and staff meetings, reporting issues, and following up with administration, team working and developing others.
